This patient underwent bilateral mastectomy and staged hybrid reconstruction. A tissue expander was first placed. This was followed by conversion to a DIEP flap and small implant for projection and volume. She underwent revisions and fat grafting in the final stage. The DIEP flap creates about 2/3 of the breast volume and is buried under the flap.
