This patient with left breast cancer underwent skin sparing mastectomy with immediate DIEP flap reconstruction using a bipedicled flap from the lower abdomen, allowing her to use all of the lower abdominal tissue for one breast, while maintaining a low abdominal scar. She was not able to spare the nipple due to tumor proximity, but will have nipple reconstruction and final tattooing to complete her result.
